I understand that some activities are considered to be at slightly higher risk for injury and that by signing this agreement, I am accepting responsibility for all risks associated by my involvement. Further, I agree to take all necessary precautions to ensure the safety of myself and my mentee while participating in higher risk activities. I also understand that Big Brothers Big Sisters of Dufferin and District can end participation of an activity any time the agency deems an activity to be unsafe or at high risk of injury to the mentee or volunteer.(Required) The purpose of this release is to facilitate activity participation that is in alignment with my mentee’s interests and match goals. Additionally, this release absolves Big Brothers Big Sisters of Dufferin and District of any responsibility should an accident occur.
